Safety forms
Incidents and occurrences of any nature must be reported to your supervisor and Risk & Safety. Use the Occurrence Report for theft, vandalism, fire, security breech, visitor incidents, environmental incidents, property damage, unusual or suspicious activity, or threats to personal safety. Deliver or email the report to the Risk & Safety Department on the same day or next business day.
For work-related injury, first aid or near miss, complete the Incident Report and forward to the risk & safety department. If the employee needs medical care, complete and provide the Clinic Authorization Form. Check for clinic locations with 24/7 care for work-related injuries that occur after hours or on the weekend. If the injury is serious contact Risk & Safety Manager Paula Bouyounes at 714-392-2294 immediately.
Injuries requiring a visit to the Workers’ Compensation Clinic must be reported to the risk & safety manager as soon as possible, but no later than the following day (if injury occurred after hours). The supervisor and injured employee complete the Employee Incident Report. Deliver or email the completed incident reports, the DWC-1 form (if applicable) and the Doctor’s Status Report to the Risk & Safety Department on the same day or the next business day.
